QUALIFICATIONS
There are certain qualifications to be met by people afflicted with
these diseases, who need help from Maps.
An evaluation
form must be completed by the patient or family member. Income guidelines are to be followed. A complete set of tax
forms for the previous year, or a bank statement for a period of 6 months.
Medical
papers and prescriptions must be completed by the patient's family doctor.
Referrals by social
workers, and other members of the health community are welcome.
